Working memory and information processing speed were examined in children with myelomeningocele and shunted hydrocephalus using the Children's Paced Auditory Serial Addition Test (CHIPASAT). The CHIPASAT was administered to 31 children with myelomeningocele and shunted hydrocephalus and 27 healthy siblings, all between 8 and 15 years of age. They also completed other standardized measures of working memory and processing speed. Children with myelomeningocele made fewer correct responses than siblings, although the magnitude of group differences declined as the rate of stimulus presentation increased. Children with myelomeningocele also made fewer consecutive correct responses and were more likely to provide correct but nonconsecutive responses, suggesting that they responded in a way that circumvents the working memory demands of the task. Standardized measures of processing speed and working memory accounted for significant variance in CHIPASAT performance after controlling for age, group membership, math skill, and general intellectual functioning. The results indicate that children with myelomeningocele and shunted hydrocephalus display deficits in working memory and information processing speed, and suggest that the CHIPASAT may provide a valid measure of these skills. Impaired imitation of skilled gestures is commonly reported in autism. Questions, however, remain as to whether impaired imitation is associated with a more generalized deficit in performance of gestures consistent with a dyspraxia and whether the pattern of errors differs from that observed in typically developing children. To address these questions, praxis in 21 high-functioning children with autism spectrum disorders (ASD) was compared with 24 typically developing controls using a traditional approach in which performance was evaluated through detailed examination of error types. Children with ASD produced significantly fewer correct responses not only during Gesture to Imitation, but also during Gesture to Command and with Tool Use. The pattern of errors in ASD was similar to that of controls with spatial errors being most common in both groups; however, body-part-for-tool errors were more common in children with ASD, suggesting dyspraxia is not entirely attributable to motor deficits. The findings suggest that autism is associated with a generalized praxis deficit, rather than a deficit specific to imitation. In a developmental disorder such as autism, the findings may reflect abnormalities in frontal/parietal–subcortical circuits important for acquisition (i.e., learning) of sensory representations of movement and/or the motor sequence programs necessary to execute them. This study directly compared how well Korsakoff patients can process spatial and temporal order information in memory under conditions that included presentation of only a single feature (i.e., temporal or spatial information), combined spatiotemporal presentation, and combined spatiotemporal order recall. Korsakoff patients were found to suffer comparable spatial and temporal order recall deficits. Of interest, recall of a single feature was the same when only spatial or temporal information was presented compared to conditions that included combined spatiotemporal presentation and recall. In contrast, control participants performed worse when they have to recall both spatial and temporal order compared to when they have to recall only one of these features. These findings together indicate that spatial and temporal information are not automatically integrated. Korsakoff patients have profound problems in coding the feature at hand. Moreover, their lower recall of both features at the same time suggests that Korsakoff patients are impaired in binding different contextual attributes together in memory. (JINS, 2006, 12, 327–336.)

Recent research has demonstrated some beneficial effects in patients with neglect using rightward shifting prismatic lenses. Despite a great deal of research exploring this effect, we know very little about the cognitive mechanisms underlying prism adaptation in neglect. We examined the possibility that prism adaptation influences visual attention by having healthy participants complete either a reflexive or a voluntary covert visual attention cuing paradigm before and after adaptation to leftward, rightward, or sham (no shift) prisms. The results for reflexive orienting demonstrated that a subset of participants with large cuing effects before prism adaptation were faster to reorient attention away from an invalid cue on the side of space opposite the prismatic shift post adaptation. For voluntary orienting, left prisms increased the efficiency of voluntary attention in both left and right visual space in participants with a small cuing effect before prism adaptation. In contrast, right prisms decreased the efficiency of voluntary attention in both left and right space for participants with a large cuing effect before prism adaptation. No significant effects were observed in the sham prism groups. These results suggest that prism adaptation may exert a variety of influences on attentional orienting mechanisms. Demographic factors and cognitive impairment have been found previously to have associations with outcome after brain injury. Kendall and Terry (1996) suggest that preinjury psychosocial functioning, neurological factors, and cognitive impairment have a direct relationship with multidimensional psychosocial adjustment, but that cognitive impairment also has an indirect relationship by means of the mediation of appraisal and coping variables. The aim of this study was to explore these theoretical relationships at very late stages of recovery after brain injury. A total of 131 participants who were more than 10 years after injury (mean = 15.31 yr) completed a neuropsychological assessment, plus outcome measures that included employment status, community integration, life satisfaction, quality of life (QoL), and emotion. Results indicated that injury severity was predictive of life satisfaction; gender and relationship status predicted community integration; and age at injury predicted employment status. Impairment in working memory directly predicted all outcomes except QoL and anxiety. An indirect relationship was also evident between working memory, life satisfaction, and depression. Results partially support Kendall and Terry's model but the variables that significantly influence outcome seem to be determined by the outcome dimensions selected. (JINS, 2006, 12, 350–358.)

Kendall and Terry (1996) include many psychosocial predictors in their theoretical model that explains individual differences in psychosocial adjustment (Lazarus & Folkman, 1984). The model depicts appraisal and coping variables as mediating relationships between situation factors, environmental and personal resources, and multidimensional outcome. The aim of this study was to explore these theoretical relationships at very late stages of recovery from traumatic brain injury. A total of 131 participants who were more than 10 years post-injury (mean = 15.31 years) completed several psychosocial measures relating to outcome dimensions comprising employment, community integration, life satisfaction, quality of life (QoL), and emotion. There was no evidence that appraisal and coping variables mediated relationships between psychosocial and any of the outcome variables. However, when appraisal and coping variables were combined with psychosocial variables as direct predictors of outcome, every outcome except employment status was reliably predicted, accounting for between 31 and 46% of the variance. Personality significantly influenced all predicted outcomes. Self-efficacy contributed to the prediction of all outcomes except QoL. Data did not support for the theory of stress and adjustment as a framework for explaining the nature of predictive relationships between psychosocial variables and very long-term, multidimensional outcome after brain injury. It remains essential to document the neuropsychological profile of acquired immunodeficiency syndrome (AIDS) dementia complex (ADC) and minor forms human immunodeficiency virus (HIV)-associated neurocognitive impairment by quantifying the magnitude of impairment across eras of treatment. Indeed, with the introduction of the highly active antiretroviral therapy (HAART), there is evidence of changes in aspects of ADC. To allow quantitative and qualitative comparisons with the HAART era studies, we developed a summary of neuropsychological performance acquired in pre-HAART era studies in advanced HIV infection and ADC. Using a meta-analytical procedure and a test nomenclature that accounts for task complexity, we found that individuals with symptomatic infection (but no AIDS) demonstrated a global mild level of cognitive impairment, except for the domains complex attention/psychomotor speed, motor coordination, and learning, which showed moderate impairment. Individuals with AIDS demonstrated a global moderate level of cognitive impairment with a predominance of deficits in attention, complex attention/psychomotor speed, learning, motor coordination, with additional deficits in verbal memory and reasoning. Individuals with ADC demonstrated the most severe cognitive disturbances in domains of learning, motor coordination, with additional deficits in verbal fluency and verbal memory. Moderate impairment was evidenced in domains of complex attention/psychomotor speed, whereas naming and visuospatial functions were relatively preserved. The profile of deficits in ADC suggests that it may not be only interpreted as a worsening form of the impairment that is seen in the AIDS and symptomatic stages of HIV disease but that there are also additional deficits suggestive of an alternate pathogenetic process(es). Observation of figural reproductions of right temporal lobe epilepsy (RTLE) patients have revealed overall configuration errors, whereas in left temporal lobe epilepsy (LTLE) patients, the global aspect of performance is generally preserved. This study compared performance of individuals with temporal lobe epilepsy (LTLE, n = 83; RTLE, n = 63) on a global/local modification of the Rey-Osterrieth Complex Figure Test (RCFT). In Phase I, neuropsychologists (n = 6) and neuropsychology fellows (n = 3) completed a survey identifying the global and local aspects of the RCFT. Questionnaire responses were used to categorize a list of global and local items (five global, five local) for re-scoring the protocols of TLE patients during study Phase II. Results showed that the RTLE and LTLE groups were not differentiated according to the global or local indices F(1,141) = .385; p = not significant. There were lower local scores for both groups in the copy F(1,142) = 5.23; p = .024, immediate F(1,142) = 445.26; p < .001, and delay trials F(1,142) = 427.82; p < .001, indicating less retention of local information over time. Results suggest general declines in figural memory for local stimulus properties in both unilateral TLE groups. However, this relationship was weakened after controlling for global and local item verbalizability. (JINS, 2006, 12, 383–390.)

Adaptations of the National Adult Reading Test (NART) for assessing premorbid intelligence in languages other than English requires (a) generating word-items that are rare and do not follow grapheme-to-phoneme mappings common in that language, and (b) subsequent validation against a cognitive battery normed on the population of interest. Such tests exist for Italy, France, Spain, and Argentina, all normed against national versions of the Wechsler Adult Intelligence Scale. Given the varieties of Spanish spoken in the United States, the adaptation of the Spanish Word Accentuation Test (WAT) requires re-validating the original word list, plus possible new items, against a cognitive battery that has been normed on Spanish-speakers from many countries. This study reports the generation of 55 additional words and revalidation in a sample of 80 older, Spanish-dominant immigrants. The Batería Woodcock-Muñoz Revisada (BWM-R), normed on Spanish speakers from six countries and five U.S. states, was used to establish criterion validity. The original WAT word list accounted for 77% of the variance in the BWM-R and 58% of the variance in Ravens Colored Progressive Matrices, suggesting that the unmodified list possesses adequate predictive validity as an indicator of intelligence. Patients with Alzheimer's disease (AD) exhibit a deficit in episodic recognition memory for odors. It is hypothesized that the higher rate of AD in women may be due to estrogen-deprivation in postmenopausal women. Research suggests that estrogen may help to minimize cognitive decline in AD as well as postmenopausal olfactory loss. The current study examined the effects of estrogen replacement therapy (ERT) on performance of a recognition memory task for olfactory and visual stimuli in women AD patients. Participants included 24 women AD patients who were ERT users and 77 women AD patients who never used ERT. Compared with the ERT non-users, the ERT users committed significantly less false-positive memory errors for olfactory stimuli, whereas performance for visual stimuli did not differentiate between ERT users and non-users. The results suggest benefits of ERT could help ameliorate the earliest symptoms of AD, olfactory dysfunction, and memory impairment. (JINS, 2006, 12, 400–404.)

Increasing evidence indicates that substance-dependent individuals (SDI) are impaired in executive control tasks relying on different systems within the prefrontal cortex (PFC). Three different functional systems have been described: the dorsolateral prefrontal cortex (DLPC), orbitofrontal cortex (OFC), and anterior cingulate cortex (ACC) circuits. Dysfunction within each PFC system is associated with different behavioral, cognitive, and emotional abnormalities. Few studies have conducted an exhaustive examination of all these different factors in SDI. In this study, SDI (including alcohol, cocaine, and methamphetamine polysubstance users, n = 35) were compared with healthy controls (n = 36) on a series of behavioral (Frontal Systems Behaviour Scale, FrSBe), cognitive (N-back, Go-No Go, and Wisconsin Card Sorting Tasks), and emotional (International Affective Picture System, IAPS) tasks, each of which was thought to tax a different component of these PFC functional systems. SDI showed greater behavioral problems in the apathy, disinhibition, and executive dysfunction subscales of the FrSBe. Behavioral deficits were significantly associated with several real-life domains in which SDI typically have problems. SDI also showed poorer performance on cognitive tests of working memory, response inhibition and mental flexibility, and abnormal processing of affective images from the IAPS. Some studies of negative priming and other tasks assumed to reflect inhibitory functions suggest a decline in inhibitory processes in Alzheimer's disease. However, none of the measures used in previous studies can be interpreted as an unambiguous reflection of distractor inhibition. The present study investigates whether reductions in negative priming associated with Alzheimer's disease reflect reduced distractor inhibition, rather than perceptual review processes. Individuals with early Alzheimer's disease were predicted to show reduced negative priming on a spatial localization task designed to provide an unambiguous measure of distractor inhibition. Sixteen clinical participants showed significantly less negative priming than old and young healthy control groups, which is interpreted as evidence for reduced distractor inhibition in early dementia. A second analysis indicated that, within the clinical sample only, negative priming effect size was significantly correlated with prime trial response speed. Clinical participants showing the least negative priming were slower to respond to an initial stimulus. The results may mean that people with early Alzheimer's disease have a reduced capacity to use excitatory as well as inhibitory processes in selection. (JINS, 2006, 12, 416–423.)

The neurocognitive impact of the clade C viral strain of human immunodeficiency virus (HIV) has not been determined. The purpose of this study was to examine neurocognitive function in southern India among individuals with the clade C virus with advanced HIV. A battery of cognitive tasks sensitive to the effects of HIV on brain function was translated and administered in Tamil and Telegu, two widely spoken languages in southern India. A sample of 30 treatment-naïve HIV-positive individuals with a median CD4 cell count of 97, and 30 age and education matched healthy controls obtained from the same region of India, were included in the study. Results revealed significant differences on most cognitive tests, with lower performances obtained by the HIV-positive individuals. These results suggest that cognitive difficulties are present among individuals with the clade C virus in India, with as many as 56% of the patients with advanced HIV meeting the criterion for impairment in two cognitive domains. Additional study is needed to determine if clade C HIV infection is more or less prone to cause neurocognitive deficit than the clade B virus. Furthermore, the impact of antiretroviral therapy on neurocognitive dysfunction in clade C virual infection needs to be determined. (JINS, 2006, 12, 424–430.)

Anorexia nervosa (AN) is a severe mental illness characterized in part by rigid thinking and ritualized behaviors involving eating and weight. Cognitive rigidity may play a role in the perpetuation of symptoms, and may provide information as to important brain-based abnormalities. Neuropsychological studies of patients with AN have shown cognitive dysfunction, but few have focused on cognitive flexibility. This study assessed set shifting in patients with AN, as a measure of cognitive flexibility. In this study, 15 patients with AN were compared with 11 healthy controls using a neuropsychological battery including the Wisconsin Card Sort Test (WCST). While patients with AN did not differ from controls on 5 measures of neuropsychological function, they made significantly more perseverative errors on the WCST, indicating a problem in set shifting. This finding suggests that patients with AN have a specific neurocognitive abnormality that may play a role in the development and persistence of this disorder. (JINS, 2006, 12, 431–435.)

Impaired recall for early items (primacy) and late items (recency) on word list recall tests are seen in Alzheimer's disease (AD). We compared conventional scoring on the Telephone Instrument for Cognitive Status (TICS) recall list with scorings based on retention-weighted recall (RWR: each item weighted by its serial position) in older adults participating in a community-based aging study. Subjects with mild AD (N = 18) did not differ from those without dementia (N = 231) with respect to recency (46% vs. 59%, p = 0.2), but had impaired primacy (2% vs. 39%, p < .001) on word recall on the TICS. RWR scoring improved the effect size (1.52 SD) compared to conventional scoring (1.08 SD). With a fixed sensitivity of 85%, specificity was lower using conventional scoring (56%) than RWR (76%) scoring. Our findings suggest that optimized RWR scoring of word list free recall can improve detection of mild AD compared to conventional scoring. (JINS, 2006, 12, 436–440.)

Recovery After Stroke. Michael P. Barnes, Bruce H. Dobkin, and Julien Bogousslavsky, (Eds.). 2005. Cambridge, UK: Cambridge University Press, 656 pp., $170.00 (HB).

Recovery after Stroke is an edited book (656 pages), and its contributors hale mostly from the United Kingdom and Western Europe. The title might suggest a focus on rehabilitation and treatment, but the contents extend well beyond, and include the neuroanatomic foundations of stroke syndromes as well as their “natural” course of recovery. The book is quite clearly intended for a general, multidisciplinary audience. As such, it covers a wide variety of topics such as spasticity/pain, imbalance and gait disorders, sleep disorders, incontinence, dysphagia, and the theory and practice of physical rehabilitation. More systemically, there are chapters addressing the principals of vocational rehabilitation, community rehabilitation, and sexual relationships after stroke.

Visual Agnosia (Second Edition). Martha J. Farah (Ed.). 2005. Cambridge, MA: The MIT Press/Bradford Books, 192 pp., $25.00 (PB).

Martha Farah published her influential volume Visual Agnosia in 1990. As she notes in the Preface to the second edition of this book, she undertook the project in the interest of “reviewing and systematizing the case literature and posing some basic questions that could, in principle, be answered by such cases.” The first edition of Visual Agnosia accomplished her objectives. From the extraordinarily rich but often confusing body of neuropsychological data, Dr. Farah developed a comprehensive account of visual processing disorders that was both true to the clinical phenomena and informed by then-current accounts of normal vision. Her short monograph quickly gained a wide audience and has become a standard reference. It has occupied a readily accessible spot on my bookshelf for years.

Fatigue as a Window to the Brain, John DeLuca (Ed.). 2005. Cambridge, MA: The MIT Press, 336 pp., $55.00 (HB).

Fatigue is ubiquitous and falls within the purview of several specialties, including neurology, psychiatry, neuropsychology, endocrinology, rheumatology, and immunology. As Simon Wessely points out in the Foreword of Fatigue as a Window to the Brain, fatigue has been virtually overlooked as an area of scientific study, because it is difficult to measure and, as a symptom, rarely aids in differential diagnosis. John DeLuca's edited book is part of the Issues in Clinical and Cognitive Neuropsychology series edited by Jordan Grafman. This is an ambitious book that examines the multidimensional and multifactorial nature of the neurobiology of central fatigue. This book advances the reader's understanding of the neural mechanisms of fatigue through review and integration of empirical data on fatigue and its cognitive correlates in neurological, medical, and psychiatric disorders.
